What is the Patient Portal?

In the Patient Portal, you can access your medical records and lab test results. Everything you need is in one place—the Patient Portal. Our staff is available to help you get set up and answer ANY questions you may have. We are here to help YOU. Just give us a call at 1-888-711-3785.

Can I fill out all of my forms before I come in?

Yes! All necessary forms are available HERE. Please fill them out and bring them with you when you come in.


How can I pay for my services?

Arbor Family Health welcomes all patients and accepts all major health and dental insurances as well as Medicare and Medicaid. Our staff is available to help you file your insurance claims, and answer ANY questions you may have. Remember, we are here to help YOU. Just give us a call at 1-888-711-3785.



What are Sliding Scale discounts?

As a Federally Qualified Health Center, Arbor offers sliding scale fee discounts for those who are uninsured or underinsured.

You may be eligible for discounted services even if you have private insurance. If you qualify, you will receive discounts on certain health care services according to the sliding scale. Sliding scale fees must be paid at the clinics, at the time that the services are rendered.

HOW TO APPLY FOR SLIDING SCALE DISCOUNTS:

You must provide proof of your household size and your total household income.

Fill out a Statement of Income Form to find out if you qualify. The sliding fee scale is based on U.S. government Federal Poverty Guidelines.

Please note that your proof of income will need to be updated annually in order for you to continue receiving discounted healthcare services.

Why do you ask so many questions in order to qualify for Sliding Scale Discounts?

We take care of everyone in need, no matter your race, income level or sexual orientation—nothing factors in to how you are treated at Arbor Health Clinics.

Because we are a Federally Qualified Health Center, we are able to offer sliding scale discounts, but federal regulations require us to ask certain questions in order for you to receive these grants.

What is medication assistance?

The cost of medications can be too high for some people to afford. That’s why we offer a Patient Medication Assistance Program to help you get medications for free or at a greatly reduced cost. See our Arbor Family Pharmacy page for MORE INFO.

This program is available to patients who do not have a prescription reimbursement plan and who qualify according to pharmacy company requirements.

Eligibility is based on income and family size.

Our staff is happy to help you find out if you qualify and help you to enroll in the program.

What is insurance assistance?

If you are eligible for health insurance under the Affordable Care Act (Obamacare) or the Louisiana Medicaid program, we can help you to enroll.

We offer Certified Application Counselors who will meet with you one-on-one to determine if you are eligible for insurance and will even help you to fill out and submit your application if you qualify. Remember, we are here to help YOU. Just give us a call at 1-888-711-3785.


What exactly is a Federally Qualified Health Clinic, and why should it matter to me?

Federally Qualified Health Clinics, such as Arbor Health Clinics, belong to an enormous network across the United States. This network of providers is connected, but we are not owned. We are a free standing, fully federally funded grant program which is more regulated than a private practice. We are highly regulated, observed and monitored by the federal government. I am worried about COVID-19. Do you have any information?

We understand that this might be a scary time for you and your family members with the coronavirus (COVID – 19) spread. There are a few things you can do to decrease your chance of getting the coronavirus:

  • Clean your hands often and ensure others do the same

  • Clean and disinfect surfaces daily

  • Stay at home when at all possible

  • Keep a few weeks of food that won’t go bad on hand

  • Make sure you have enough of your medicines on hand

Call your pharmacy if you will need a refill in the next month. Please take all of your medicines to stay healthy. Please call one of our offices if you have any questions.

IMPORTANT: If you think you have been exposed to the coronavirus, or have symptoms such as a cough, fever, or shortness of breath, please call an Arbor Family Health Clinic and we will advise on next steps.
