    My most important message is that we don’t want the fear of not knowing something prevent you from getting services you need and may qualify for. No question is too small or silly. No amount of help in acquiring these services is something to be embarrassed about. We are here to help in any way possible!

    Obtaining phones: using companies like Assurance Wireless, our care coordinators may be able to get cell phones for patients with low income or using Medicaid services.

    Workforce preparation: The Way to Work Program allows care coordinators to work with patients to prepare them for job interviews, network with local job opportunities and oversee their immersion into the workforce

    Filling out and submitting applications: this assistance is available when applying for SNAP (Supplemental Nutrition Assistance Program/Food Stamps), Section 8 Affordable Housing, Medicaid, Medical Transportation services delivered through Medicaid (to and from medical appointments)

    Whatever the need may be, we want to provide the best and most complete assistance to our patients!

    Hello my name is Lerae Wells. I am the OUTREACH COORDINATOR at our Innis location. I have been in the medical field for 4 years. I take PRIDE in working with people and making sure that my patients feel comfortable knowing that I am there. My main message to our patients is that whatever the circumstances may be, I am here for YOU. My goals and values are to always go above and beyond to help out patients/community.
